We will try to respond to your inquiry within 24 hours. Your question will be answered by a senior officer of Hospice Foundation of America. Certain complex questions may be referred to one of our subject matter experts. Your response may be delayed somewhat during the referral process. You may receive an answer directly from one of the consultants. This project is provided through the support of a grant from the Centers for Medicare and Medicaid Services (CMS) to support hospice and end-of-life care outreach and education.

CMS funds of $571,000 with HFA in-kind services of $5,710 are funding a variety of outreach and educational programs.
